What to bring.
What do I need to request blood?
The treating doctor's requisition form with the patient's details, and the patient's blood sample as advised by the hospital, so that grouping and cross-matching can be done.

Can I donate in place of the blood my patient receives?
Replacement donation by family and friends helps keep the blood centre stocked for everyone. Please ask our staff when you call.
